  • Our family needs help now! How do we get started?

    In order to help you in the fastest way possible, you need to follow certain steps. These steps are outlined on our Families page.

    Your first step is to call PerformCare, our contracted systems administrator, at 1-877-652-7624. Children are referred to FACT through PerformCare, the contracted systems administrator, which does an assessment of the child’s needs. PerformCare will initially interview the caregiver on the telephone to determine the severity of the problem. If the family requires care management assistance, a clinician will meet with the family to do a compete evaluation of needs. If the needs of the family rise to an intensive level, they will be referred to FACT. FACT will contact the family within 24 hours.

  • Do you only serve Union County families?

    Yes. FACT is Union County's exclusive Care Management Organization (CMO) and is only able to serve residents of Union County.

    If you live outside of Union County, you should look for the CMO serving your area. There are Care Management Organizations dedicated to each of the following counties: Bergen, Burlington, Camden, Cape Atlantic, Essex, Hudson, Mercer, Middlesex, Monmouth, Morris/Sussex, Ocean, Passaic, and TriCounty (Hunterdon Somerset Warren).

  • What does FACT do?

    Fact helps families form a Child and Family Team of Support around a child/adolescent in crisis with people who truly know and care about their future.

    We also help families learn how to use and manage services that are available to them in their community.

  • Where can I see your list of community based care providers?

    You can go to our sister website UnionResourceNet.org to find our comprehensive list of community based support, resources, and services for children and families.
